Norse Personal Training Under 16 Yrs Old Waiver Document

Thanks for wanting your under 16 year old to participate in Norse Personal Training/Boot Camp sessions. This agreement outlines the session guidelines and should be completed before your Under 16 year old's first training session.

Norse Boot Camp Sessions Guidelines

Please read each statement below, complete your details and hit the submit button at the bottom of the page to acknowledge your understanding. 

  • Under 16 PT sessions/Boot camp classes are non-refundable & Non-transferable 

  • You may participate only after payment

  • The PARQ has been completed and reviewed

 

Waiver & Release Agreement

You are aware that in your capacity are a parent/guardian, your under 16 year old child's participation a sport or physical exercise may result in accident or injury, and assumes the risk connected with such participation. The child you are responsible for and whom is a participant in  Norse Boot Camp/Personal Training sessions is in good health and suffers from no physical impairment, which would limit their participation in this session or any physical impairments disclosed on PARQ form. In your capacity are a parent/guardian, you specifically agree that Norse Personal Training shall not be liable for any claim, demand, and cause of action of any kind resulting from or related to your child's participation in the Personal Training/Boot camp sessions.
